In consideration of the above-named child being allowed to participate in any athletic/sports event sponsored by DRiB-L, Inc, in any related events and activities, and intending to be legally bound, the undersigned:

1.Acknowledges and fully understands that each child will be engaging in activities that involve risk of injury, including injuries which might result not only from their own actions, inactions or negligence, but also from the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not know to us or not reasonably foreseeable at this
time.

2. Assumes all the foregoing risks and accepts personal responsibility for the damages which may arise from any such injury.

3. Releases, waives, discharges and covenants not to sue DRiB-L, Inc. and its administrators, directors, officers, representatives, agents, and employees, as well as other participants of the event, including sponsoring agencies, sponsors, advertisers, an if applicable, owners and lesser of premises used to conduct the event, all of whom are hereinafter are referred to a “releases”, from any and all liability to the undersigned, his or her heirs and next of kin, and/or the above named child for any and all claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or any part by the Negligence of the releases or otherwise.

I hereby pledge to provide positive support, care, and encouragement for my child participating in youth sports by following this Parents’ Code of Ethics:

I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice or other youth sports event. I will place the emotional and physical well being of my child ahead of my personal desire to win.  I will insist that my child play in a safe and healthy environment.  I will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all.  I will demand a sports environment for my child that is free from drugs and alcohol and will refrain from their use at all youth sports events.  I will remember that the game is for youth - not adults.  I will do my very best to make youth sports fun for my child.  I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability. I will help my child enjoy the youth sports experience by doing whatever I can, such as being a respectful fan, assisting with coaching, or providing transportation.
